In order to be able to download any of the WHODAS versions or the Training
Materials you need to accept the Review Agreement and register yourself to our
database.
Review Agreement for WHODAS II materials
WHO is hereby prepared to make available to you the World Health Organization Disability Assessment Schedule II and/or related materials (hereafter referred to as "WHODAS II").You hereby agree that:
1. You are free to examine the WHODAS II, which is currently under development and not yet available for general release and distribution.
2. You shall not reproduce the WHODAS II and shall in no event distribute copies of the WHODAS II to third parties by sale, rental, lease, lending, or any other means.
3. You shall not modify, abridge, condense, translate, adapt, recast or transform the WHODAS II in any manner or form, including but not limited to any minor or significant change in wording or organization, or administration procedures, of the WHODAS II.
4. You shall not use the WHODAS II for any purpose (research, teaching, or clinical) unless agreed in writing by WHO. If you would like to use the WHODAS II in research, you shall complete and sign a User Agreement Form* and return it by mail or fax to WHO.
5. You shall be responsible for any breach of this agreement and you agree, at its sole expense, to take all responsible measures (including but not limited to court proceedings) to restrain the prohibited or unauthorized use of the WHODAS II.
6. WHO may terminate this Review Agreement at any time, in any event. Should WHO terminate this Review Agreement, you shall immediately destroy or return all copies of the WHODAS II. In the event of such termination, all other collateral materials shall be destroyed and no copy thereof shall be retained by you. Notwithstanding the return or destruction of the WHODAS II and its collateral materials, you will continue to be bound by the terms of this Review Agreement.
